Prepare for Your Visit
Sports physicals are covered by most insurances when included in routine well-child exams.
To streamline your appointment:
- Bring any required forms provided by your school or sports organization.
- The required forms will be completed by a provider during a well-child exam.
- Wear comfortable clothing suitable for light physical activity.
- Come prepared with your medical history and a list of any current medications.
MSHAA Sports Physical Forms
The Michigan High School Athletic Association (MHSAA) requires all students who participate in sports to have a pre-participation sports physical. A physical examination must be completed on or after April 15 to be considered a valid physical for the upcoming school year.
